Instructions

Any registered voter- regardless of party registration- can sign John's petition to get on the ballot. 

For signers:

 

• Print petition signature sheets on standard 8 ½ x 11 inch paper

• Legibly write the following on the signature sheet (only sign once):

  1. Sign your name as it appears on your voter registration

  2. Date (month, day, year)

  3. Residential address (per voter registration)

  4. Zip Code

  5. Congressional District

  6. Printed name (as it appears on voter registration- nicknames or initials will not count!

For circulators:

No more than 15 people can sign any one petition sheet. Each signer to a petition sheet must reside in the same county in Missouri. 

 

Once a petition sheet is filled, the Circulator of the petition MUST

  1. Fill in the County where the petitions were collected

  2. Sign the Circulator Affidavit at the bottom of the signature sheet. The Circulator’s date must be the same or later than the date of the last petition signer.

 

The Circulator Affidavit must be witnessed and notaraized by a sworn notary public registered in the state of Missouri.

Once your form is complete, please mail the completed petition sheet(s) to:
